Cost is $50 per month. Classes are held once a week on Thursdays for 30 minutes at 11:00 AM. Sign-ups are available at any time.

Little Tennis Academy Registration Terms

Sickness

I agree not to bring a sick child to class. I understand it is my duty to ensure that medications my child requires have been given prior to lessons. I represent that my child/children are healthy and able to participate in lessons. I acknowledge that The Little Tennis Academy LLC will not administer medications except in emergencies as provided in the Medical Release and Waiver below.

Registration Fees

I understand that there is a once per school year registration fee of $15 per child

Class Fees

I understand that the Class Fee(s) must be paid monthly. I understand that after LTA accepts the Application for Registration, there is no refund. There will be no re-scheduling of lessons or prorating due to vacation or illness.

Medical Release

I hereby give permission to The Little Tennis Academy LLC to render temporary first aid to the above- named student(s)/child(ren) in the event of any injury, illness or emergency. I consent to have my child participate in lessons offered by The Little Tennis Academy LLC, a program of physical activity including, but not limited to basic tennis exercise movements, running, jumping, swinging a tennis racquet, hitting tennis balls, etc. being fully aware of the risks and possibility of injury involved in relationship to tennis activities, children at play, ingress and egress and use of LTA facilities, and interaction with staff and instructors. I further waive and release all rights and claims for injury or damages, including but not limited to negligence, that I, my spouse, and/or my child(ren), or any other party claiming through me or on my behalf or that of my child/children may have against The Little Tennis Academy LLC, it's officers, members, employees, instructors and volunteers, and the facility at which LTA sessions are held, where ever and howsoever those claims may arise.

Discipline

I understand that it is the policy of LTA that if a student is unwilling to cooperate, becomes a negative distraction or acts in an unsafe manner in class the following positive discipline will be applied: Time-out and if that does not cure the issue, the child will sit-out the remainder of the class. The Responsible Party will also be notified.
